Neoadjuvant Immunotherapy for Patients with dMMR/MSI-High Gastrointestinal Cancers: A Changing Paradigm

نویسندگان

چکیده

Immune checkpoint inhibitors have revolutionized the management of mismatch repair-deficient (MMR-D)/microsatellite instability-high (MSI-H) gastrointestinal cancers, particularly colorectal cancer. Cancers with MMR-D/MSI-H genotype often carry a higher tumor mutation burden frameshift alterations, leading to increased mutation-associated neoantigen (MANA) generation. The dramatic response seen immune (ICIs), which are orchestrated by MANA-primed effector T cells, resulted in rapid development these novel therapeutics within landscape MSI-H cancers. Recently, several clinical trials utilized ICIs as potential neoadjuvant therapies for cancers and demonstrated deep pathological responses, creating opportunities organ preservation. However, there challenges use certain disease types due risk overtreatment that can be cured through surgery-only approach. In this review article, we discuss approaches ICI therapy patients including those oligometastatic disease. We also elaborate on utilization provide further insight into changing treatment paradigm

Immunotherapy in Gastrointestinal Cancers

Gastrointestinal cancers represent a major public health problem worldwide. Immunotherapeutic strategies are currently under investigation in this setting and preliminary results of ongoing trials adopting checkpoint inhibitors are striking. Indeed, although a poor immunogenicity for GI has been reported, a strong biological rationale supports the development of immunotherapy in this field. The...

Neoadjuvant Immunotherapy in High Risk Patients with Cutaneous Melanoma: A Novel Approach

Cutaneous melanoma is an immunogenic tumor, but it seems to be very heterogeneous. Utilizing patient own tumor, before it excision, as the source for tumor-specific antigens, intratumoral administration of low dose of GM-CSF weekly in dermal and subdermal metastases did result in over 50% response rate. Failure to establish complete tumor response (CR), low weekly dose of IL-2 was substituted a...

Role of Frizzled receptor expression on patients’ survival with gastrointestinal cancers: A systematic review with meta-analysis

Background: Frizzled receptors (FZD) play a pivotal role in the initiation and progression of a wide array of cancers. Dysregulated expression of FZD receptors is correlated with higher metastasis and invasive potential, as well as short survival in many malignancies. In this meta-analysis, we aimed to verify the prognostic value of FZD receptor expression on patients’ survival with different t...

Current Status and Perspective of Immunotherapy in Gastrointestinal Cancers

Cancer immunotherapy is at dawn of the Renaissance after the Medieval Dark Ages. Recent advances of understanding tumor immunology and molecular drug development are leading us to the epoch of cancer immunotherapy. Some types of immunotherapy have shown to provide survival benefit for patients with solid tumors such as malignant melanoma, renal cell carcinoma, or non-small cell lung cancer. Sev...
